There is one on the BMD index on Ancestry which might be yours.

    Apr-May-Jun 1889
    Registration district - Faringdon
    Volume Number: 2c
    Page Number: 461

There are 5 names on the page so presumably one has been mistranscribed:
    Eliza Ann Collins
    Emma Crook
    Philip Harris
    James Tomlin
Henry Watcroft - a correction for this has been submitted - Henry Walcraft
