I recently received a copy death certificate with cause of death "nitral incompetence". Would anyone know what this means? She died in 1934 and there was no post-mortem.

I've just Googled "nitral incompetence" and it would appear to be some sort of heart disease where the nitral/mitral valve in the heart ceases to work.
