You don't have to change your name by deed poll. You can just change it!
Nowadays, with so much fuss about identity and fraud and terrorism etc, and when people routinely have bank accounts and passports and driving licences, it is more likely there would be an official record of changes.

There doesn't need to be any formal procedure. The mother can just tell people the child's surname is Y instead of X.
Anyone can use any surname they want to, provided they are not using that surname for fraudulent purposes, although today that might cause problems with banks and so on.

If parents were never married, child's birth would be indexed in both names anyway.

Not sure that is a quick question, lol!
What she needs to do is to connect what it says on her birth cert (see my message immediately above this one) with what documentation she has now.

My mother tried to get a passport, but because she was unable to marry my father (no divorce for him) she changed her surname. She had to go to a solicitor amd make a declaration, showing her birth certificate, that she had changed her name. She got her passport OK.Wendy

If her name was changed when she was 2 or 3 there will be loads of things with her name on - school records, medical records, employment records. All of these will show that she has been known as Jane Doe or whatever for years.
If Doe is her mother's maiden name it should be on the birth cert. When she was 16 she should have got a National Insurance Card which would prove her name and age, surely?~ with love from Little Nell~Chowns, Dunt, Emms, Mealing, Purvey & Smoothy
