It's a beautiful page-turn set-up of the West Herts (Garston, near Watford) Crematorium's Book of Remembrance.
It's not searchable by name (as far as I can tell at the moment) but you can jump to a particular volume and pick a date.
The earliest date seems to be 1 Jan 1959.
